Sweepstake Prizes:
One winner will receive the Grand Prize which is vacation golf getaway for two valued at $4,000 and includes:
– a four (4) night stay in a suite at the historic Wigwam Resort located in Phoenix, AZ;
– three rounds of golf at any of the three Robert Trent Jones designed championship golf courses;
– a 2 hour golf lesson at The Plane Truth Golf School;

Daily Prizes:
Sponsor has reserved twenty-eight $25 Shop Etc. Mall Gift Cards to be awarded to one person after each of the Daily Promo-Dates.
The total Approximate Retail Value (“ARV”) of all prizes awarded is $11,846.88.
The following restrictions apply to the Grand Prize winner’s vacation getaway: Airfare may not exceed $800 per ticket. All tickets and hotel stay will expire by or before one year from the date prize is awarded to winner. Restrictions apply including expiration dates and blackout dates for hotel stay and travel. Winner is responsible for any costs/expenses not specifically listed above including, but not limited to, taxes, meals, drinks, insurance, airport transfers not stated, car rentals, taxi, gratuity, baggage fees, communication charges, and all personal expenses and incidentals. The Sponsor or its’ prize provider determine the hotel room and air travel, this determination is based on availability and is not negotiable. All gift certificates and gift cards are subject to the terms and conditions of the gift card issuer, including service fees. Hotel stay and travel must be used prior to expiration date, any unused portion of the prize will be automatically forfeited. Winner must make arrangements directly with Sponsor’s prize provider, Sponsor will provide all applicable information for the winner to correspond with Sponsor and its’ Agents concerning prize redemption details. Once booking is finalized, no deviations will be allowed for any portion of the prize. The Grand Prize must be accepted as awarded and may not be sold or transferred and is not convertible to cash. Sponsor or its’ Agents reserve the right to require additional documentation from winner and their guest. Additional prize restrictions and terms may be imposed by prize providers. The Sponsors reserve the right to substitute a prize in whole or in part in the event that all or any component of the Grand Prize is unavailable. All properly claimed prizes will be awarded. All travelers must possess required travel documents. Grand Prize Winner’s guest, if any, must be 18 or under the legal guardianship of the Grand Prize Winner, and travel on the same itinerary. The actual retail value of the Grand Prize may depend on Winner’s point of departure/return, winner will not receive compensation for any difference between actual value and stated value. Other restrictions may apply. Prizes consist only of those items specifically listed. All prizes are awarded without warranty, express or implied, of any kind. Sponsor reserves the right to substitute a prize with a prize of equal or greater value.
Winners of the Sweepstake Prizes will be selected in a random drawing held on or about June 22, 2012. Winners of the Daily Prizes will be selected in a random drawing held after the Daily Promo Date and within 4 days of the close of the Promotional Period.
Claiming Prizes: Winners of the Daily Prizes will receive the gift card in the U.S. Mail or via a reputable carrier. Daily Prizes will be shipped within two (2) weeks of the end of the Promotional Period. Winners of the Sweepstakes Prizes will be notified by email, text, or phone and each unverified Sweepstake Prize winner must provide Sponsor with a valid email address, via email, within 7 days of the first notification attempt made by the Sponsor or the prize will be forfeited and an alternate winner selected. Winners of the Sweepstake Prizes will be required to sign and return an Affidavit of Eligibility & Liability and Publicity Release (“Release”) & W9 form which must be received by Sponsor or its agent within 10 days of the date it was emailed or the prize will be forfeited and an alternate winner selected. Return of any prize notification as undeliverable will be forfeited and an alternate winner will be selected. The Grand Prize winner may be required to provide Sponsor with additional documentation and correspondence in order to receive prizes.
