This class action settlement covers people whose private information was potentially affected in a data security incident at Vida Y Salud-Health Systems, Inc. in October 2025. Information that may have been accessed includes names, Social Security numbers, claims details, diagnosis or condition information, and credit card or bank account numbers. The company denies wrongdoing, and the court has not decided the case. Eligible people who submit a valid and timely claim form may receive benefits, including a cash payment. The claim can be filed online through the settlement website, or by downloading a paper form and mailing it to the settlement administrator. You can also call or email the administrator to request a paper form. The claim deadline is January 18, 2027. If you do nothing, you will not receive a payment and will give up the right to sue over the covered claims. Opting out or objecting has an earlier deadline of January 4, 2027.
