It asked for my state and zip, but no street address. Then the next page said they are on their way. Weird.

Mine seemed to go through ok. It said they are on the way. The only thing I don’t get is they didn’t ask for any address.

The form popped up. I had to reload the page several times.. It kept saying wrong captcha it wasn’t wrong. Finally it went through, and it said I was not eligible.

I’m in ut and had no problem using the 1st code. Said my glasses were on the way. Is there a reason a different code is listed for ut now?

I think they use a reverse phone directory on your home phone number and zip code to check for your address; apparently you can be found.
