(2) Complete the entry form by entering the following information: your first name; last name; email address; home phone number with area code (cell phone number is optional), street address, city, state or province, zip or postal code, country, date of birth, and verification that you are at least 18 years of age and have read and accept the Official Rules.
(3) Submit an essay of no more than 300 characters explaining what winning a $5,000 Rite Aid gift card would mean to you.
(4) Upload a recent photo of yourself.
