Complete all required fields on the online Sweepstakes entry form and follow the instructions to upload either: a digital photograph of entrant as a baby or a digital photograph of entrant’s baby-aged child(ren) or entrant with their baby-aged child(ren); All photographs must include only the entrant and/or a baby for which entrant is the parent or legal guardian. Photographs should be at least 640 x 480 pixels. The following photo formats are acceptable jpg, tiff or bmp.

 Photograph Submission Guidelines: Entrant’s photograph and all contents of entrant’s photograph(s), including but not limited to, content, text, images, captions and descriptions must be completely original to entrant and must not have been taken or composed, in whole or in part, from any other source other than entrant and must not have been released commercially in order to be eligible for submission to Sweepstakes.

 Limited to one (1) entry per natural person. Entrant can submit up to three (3) photographs but will only receive one (1) entry, regardless of the number of photographs submitted.

 There will be one (1) Grand Prize Winner, two (2) First Place Winners, and one-hundred (100) Runner-Up Winners selected in a random drawing from among all of the Eligible Entries on or about February 20, 2012, at the office of the Administrator. The potential Winners will be notified by email shortly thereafter and will be required to confirm his/her address and phone number within five (5) days from when initial email is sent.

 Grand Prize (1): Grand Prize Winner will receive one (1) $1,000 Visa Gift Card.

 First Place Prizes (2): Each First Place Winner will receive one (1) $500 Visa Gift Card.

 Runner-Up Prizes (100): Each Runner-Up Winner will receive one (1) Gerber Life Crayon Calculator, with an approximate retail value (“ARV”) of $2 each.

 ** For every photo uploaded to the Project Open Book mosaic, Gerber Life Insurance will donate $1 to Reading Is Fundamental, up to a total maximum donation of $10,000.
