Prize: (1 winner) 3-day/2-night trip for two (winner and guest) to New York, NY to attend the 2010 US Open Tennis Championships. Prize includes round trip coach air transportation for two persons (winner and one (1) guest) from the major commercial airport nearest winner’s residence within the U.S. to New York, NY (if Winner resides within 200 miles of New York, NY, sponsor may substitute ground transportation for air transportation and any difference in prize value will not be awarded), two (2) nights’ standard hotel accommodations (single room, double occupancy), ground transportation to/from airport in New York, and two (2) tickets for both the day and night sessionss on 9/04/10 to the US Open Tennis Championships. Winner and guest must be available to travel 9/03/10 through 9/05/10 or prize will be forfeited in its entirety. Approximate retail value (ARV): $4,900.00. Actual value of prize may vary depending upon prize winner’s point of departure and airfare fluctuations. Any difference between the stated approximate retail value and the actual value of the prize will not be awarded. Selection of airline (or ground transportation, if applicable) and hotel are solely within Sponsor’s discretion. All other expenses on the receipt and use of the prize not specifically mentioned herein, including but not limited to tips, gratuities, required ground transportation to/from hotel to tournament venue, all meals and beverages, and other miscellaneous expenses, are solely the responsibility of the winner and winner’s guest. Prize is subject to government restrictions, and regulations. Other restrictions may apply. Winner and guest must travel on the same itinerary. Seat selection is subject to availability, in Sponsor’s sole discretion. Sponsor is not responsible for cancelled, delayed, suspended, or rescheduled events, including flights, beyond its control. Winner and guest are solely responsible for obtaining all necessary identification and/or travel documents required prior to travel (e.g., a valid U.S. driver’s license). If winner is eligible, but a minor in his/her state of residence, the travel guest must be the winner’s parent or legal guardian. If guest is a minor in his/her state of residence, he/she must be accompanied by his/her parent or legal guardian who must be the winner. Minors must be accompanied at all times during trip (including but not limited to, in-flight, hotel stay, and all prize-related events) by minor’s parent or legal guardian. Guest has no independent right or claim to any prize. Any portion of a prize not accepted by winner will be forfeited. Prize is non-transferable and no substitutions will be made except as provided herein or at Sponsor’s sole discretion. Citizen is not responsible if US Open is rained out or postponed, or cancelled after ticketing of prize for any reason, in whole or in part. In that event, winner will receive remainder of the prize. Winner must comply with these Official Rules. Winner will be notified by overnight mail on or about 8/07/10 and will be required to execute and return an Affidavit of Eligibility and Liability/Publicity Release (where permitted) within five (5) days of notification (first attempted delivery). If winner is eighteen (18) years of age, but an eligible minor in his/her state of residence, prize will be awarded to a parent/legal guardian of winner who must execute and return any required documents as noted previously in these rules and accompany the winner as his/her guest. Guest of winner will also be required to sign an Affidavit of Eligibility and Liability/Publicity Release (where permitted) before any ticketing of travel occurs. If a prize, or any portion thereof, cannot be awarded for any reason, Sponsor reserves the right to substitute prize with another prize of equal or greater value. All federal, state, and other taxes, are solely the responsibility of the winner. Winner will be issued an IRS form 1099 for the prize value over $600.00. Winner grants (and agrees to confirm this grant in writing, if requested) permission for the Sponsor and Amazon to use his/her name, photograph, likeness, statements, voice and address (city and state) on a worldwide basis, and in all forms of media, in perpetuity, without notice or further compensation except where prohibited by law. Failure of winner to comply with the above prize claim requirements for any reason, or the return of any prize notification or prize as undeliverable, or the determination that the winner is ineligible to receive a prize, may result in forfeiture of the prize. Sponsor is not responsible for any change of e-mail, mailing address and/or telephone number of entrants. If a winner is found to be in violation of any of these Official Rules, he/she may be required to forfeit his/her prize.
