The Verizon “Made for Mom” Sweepstakes

Each entrant will be required to
provide the entrant’s name, date of birth, street address, telephone number, e-mail
address and such other information as Sponsor may require. Once an entrant has
completed the Online Entry Form, the entrant must then build his or her “Grand
Prize” by choosing options from within each of the categories made available by
Sponsor on the Website, consisting of one (1) tablet or device, three (3)
accessories and one (1) trip (all fully described in Section 4 below). Upon
completion of the Online Entry Form and building his or her “Grand Prize” each
eligible entrant (“Entrant”) will receive one (1) Sweepstakes entry. Once a “Grand
Prize” selection has been completed and submitted, it cannot be changed. The
Online Entry Form and all “Grand Prize” selections must be completed and
received by Sponsor by 11:59 PM ET on May 8, 2014. There is a limit of one (1)
entry per person and per email address per day.

PRIZE: One (1) winner will receive one (1) Grand Prize. The Grand Prize
consists of the “Grand Prize” selections chosen by the winner upon entering the
Sweepstakes. The “Grand Prize” selections include:
(A) one (1) tablet or device (Verizon Wireless service not included), that the
winner selected as part of his or her “prize package” as noted in chart below;

(C) One trip selected by the winner from among one of the four (4) following trip
options:

(i) “Shopping Spree in NYC”: round trip, coach-class air transportation
for the winner and one (1) guest between the major airport nearest to
the winner’s home and a New York City area airport determined by
Sponsor; a three (3) consecutive night stay for the winner and one (1)
guest at a four-star hotel determined by Sponsor (one (1) standard
room, double occupancy; room rate and taxes only; size and type of
room determined by Sponsor); ground transportation for the winner
and guest between the airport and hotel; a river dinner cruise for the
winner and guest; and $3,000, awarded in the form of a check or
another method determined by Sponsor (ARV: $6,200);
(ii) (ii) “Ultimate Rejuvenation in Tucson, AZ”: round trip, coach-class
air transportation for the winner and one (1) guest between the major
airport nearest to the winner’s home and a Tucson, Arizona area
airport determined by Sponsor; a three (3) consecutive night stay for
the winner and one (1) guest at a spa/resort hotel determined by
Sponsor (one (1) standard room, double occupancy; room rate and
taxes only; size and type of room determined by Sponsor); a rental car
with unlimited mileage for use by the winner within the greater
Tucson area during the trip (provided the winner or guest satisfies the
rental car company’s standard requirements); spa services and meals
for the winner and guest chosen from among options provided by
Sponsor; and $600, awarded in the form of a check or another method
determined by Sponsor (ARV: $6,200);
(iii) (iii) “Sonoma, CA Wine Lover’s Package”: round trip, coach-class air
transportation for the winner and one (1) guest between the major
airport nearest to the winner’s home and a Sonoma, California area
airport determined by Sponsor; a three (3) consecutive night stay for
the winner and one (1) guest at a four-star hotel determined by
Sponsor (one (1) standard room, double occupancy; room rate and
taxes only; size and type of room determined by Sponsor); a rental car
with unlimited mileage for use by the winner within the greater
Sonoma area during the trip (provided the winner or guest satisfies
the rental car company’s standard requirements); a gourmet dinner for
the winner and guest; a The Royal Treatment Day-Spa, which
includes two (2) 60-minute treatments of your choice, Spa gift and a
glass of sparkling wine or cider ; and $1,000, awarded in the form of
a check or another method determined by Sponsor (ARV: $6,300);
(iv) (iv) “Bring Out Your Inner Child in Orlando, FL”: round trip, coach
class air transportation for the winner and one (1) guest between the
major airport nearest to the winner’s home and an Orlando, Florida
area airport determined by Sponsor; a three (3) consecutive night stay
for the winner and one (1) guest at a four-star hotel determined by
Sponsor (one (1) standard room, double occupancy; room rate and
taxes only; size and type of room determined by Sponsor); a rental car
with unlimited mileage for use by the winner within the greater
Orlando area during the trip (provided the winner or guest satisfies
the rental car company’s standard requirements); multi-park passes
for the winner and one (1) guest; and $1,000, awarded in the form of
a check or another method determined by Sponsor (ARV: $5,700).

The total ARV of the Grand Prize is between $7,700 and $8,300.
