Grand Prize: (10 winners) Trip for winner and one guest to New Orleans, Louisiana April 23-29, 2009. Trip includes round-trip coach airfare for winner and one guest from a major airport near winner’s residence to New Orleans, LA, lodging for 6-nights (double occupancy accommodations selected by Sponsor, subject to availability and restrictions), two admissions to the Jazz Festival black-tie Gala, Jazz Festival Gala passes valid during trip, two admissions to The National World War II Museum and $500.

OR, if winner is unable to travel on specific dates, a 6-night trip may be taken to New Orleans during 2009 (including round-trip coach airfare for winner and one guest from a major airport near winner’s residence to New Orleans, LA, lodging for 6-nights (double occupancy accommodations selected by Sponsor, subject to availability and restrictions) and $500). Approximate Retail Value (ARV) of trip for two is $3,900. All travel must be completed by December 30, 2009, and must be arranged through Sponsor or its agency at least 45 days prior to trip. Potential Grand Prize winners will be notified by electronic mail (email) and confirmed via certified mail or other overnight delivery method at the address provided on the entry form, and will be required to complete, sign, notarize and return an Affidavit of Eligibility and Liability/Publicity Release which must be received by Sponsor within seven (7) days of the date on the notification or prize will be forfeited and an alternate potential winner selected. Each guest of each Grand Prize winner must complete, sign and return a Liability/Publicity Release prior to issuance of travel documents. If any guest is a minor, such documents must be signed by his/her parent or legal guardian.
