An individual may only win one (1) Prize Package during the Sweepstakes Promotion Period.
Grand Prize
The Grand Prize winner will be awarded a spot in the ‘TriWest Healthcare Alliance Shot of Glory’ contest on February 2nd, 2011. Hit a hole in one and you could win $1,000,000*. A round Trip Flight Voucher compliments of US Airways (Round trip ticket is valid for travel between any two US Airways cities in the continental US only. Travel is not valid on codeshare flights) and four General Admission tickets to the Waste Management Phoenix Open. These will be awarded to the person that has invited the largest number of users to the sweepstakes and have entered the sweepstakes.
*If any participant hits a hole in one, they will win One Million Dollars. That individual will receive $500,000, their charity of choice will receive $250,000 and the military charity they chose receives the remaining $250,000. It will be paid in the form of a multiyear annuity.
2nd Prize
