Grand Prize: (1 winner) 3-day/2-night trip for two to New York City to attend a Sponsor-specified taping of The Dr. Oz Show during Spring 2014 (ARV: $5,000). In addition, a $100,000 donation will be made in the Grand Prize Winner’s name to the health or wellness charity specified in the Winner’s Essay (subject to Sponsor’s final approval of charity). Selected charity must be a recognized IRS 501(c)3 charity in good standing and must be physically located within the 50 United States or the District of Columbia, at the time of Winner determination and prize award. Charity will be required to provide proof of 501(c)3 status. Sponsor will make donation directly to the charity; Grand Prize Winner may not take tax deduction for donation portion of the prize. Grand Prize Winner and guest must agree to depart and return on the Trip dates specified by Sponsor. Dates of Trip departure and return are subject to change. Trip includes round-trip coach air transportation from major airport nearest Winner’s home, standard hotel accommodations (one room/double occupancy), lunch for winner and travel companion on day of show taping, ground transportation to/from airport/hotel, and a $500 Sponsor-specified gift card. Terms and conditions on gift card apply. Grand Prize Winner and guest are solely responsible for all other expenses not specifically set forth herein, including but not limited to additional meals, additional transportation, baggage fees, upgraded room accommodations, souvenirs and other incidentals and items of a personal nature. Date of The Dr. Oz Show taping winner/guest will attend will be specified by Sponsor. Sponsor is not responsible for lost, mutilated or stolen travel documents. The actual retail value of the prize will depend on Winner’s point of departure/return and dates of travel. If the Grand Prize Winner lives within a fifty (50) mile radius of New York City, no air transportation will be provided; only ground transportation will be provided. Grand Prize Winner and guest must travel on same itinerary. In the event The Dr. Oz Show is cancelled or postponed for any reason, Sponsor will have no obligation to award compensation in lieu thereof but the remainder of the trip will be awarded. Children under 18 years of age are not permitted to attend the taping of The Dr. Oz Show. Prizes consist of only those items specifically listed as part of the prize. Prize may not be transferred, sold or offered for sale
