Five (5) Grand Prizes – A trip for two (2) to the NASCAR Pure Michigan 400 Race in Brooklyn, MI on August 26-28th, 2016 for the Grand Prize winner and his/her favorite United States military veteran. Trip includes:
• round-trip coach airfare for two (2) people from major airport nearest winner’s residence to Brooklyn, Michigan,
• 1-nights hotel accommodations (one room, double occupancy) at a Sponsor-specified hotel in Brooklyn, MI,
• two (2) Hot Pass tickets to the Pure Michigan 400 Race on August 28th, 2016,
• transportation to/from airport/hotel/race.
Travel companion must be eighteen (18) years of age or older (or 19 or older if a resident of AL or NE) and a U.S. Military Veteran as of the date of departure and must travel on same itinerary and at the same time as the winner. Travel companions must execute liability/publicity releases prior to issuance of travel documents and will berequired to show proof of military veteran status.
ARV: $3,435.00 each. ARV of ALL Grand Prizes for this portion is: $17,175.00.
PLUS: Grand Prize winner may designate any US armed service veteran to receive FREE dental care at an Aspen Dental-branded practice supported by Sponsor for up to $10,000 in dental services. The dental services will be provided solely by such dental practices through their designated employees (“treating clinician”) after such treating clinician conducts an independent clinical assessment of the service veteran. The dental care may be withheld or delayed based upon the treating clinician’s judgment, the veteran’s health or any other relevant factor as determined by the treating clinician. All services must be completed by May 15, 2017. In the event the Grand Prize winner resides 75 miles away from the nearest Aspen Dental-branded practice supported by Sponsor, he/she will receive a $1,500.00 check as a lesser cash option made out to their designated US armed service veteran in lieu of the dental services.
ARV is up to $10,000.00. ARV of ALL Grand Prizes for this portion is: up to $50,000.00.
MUST HAVE PROOF OF MILITARY OR FORMER MILITARY STATUS TO RECEIVE THE DENTAL SERVICE/CASH OPTION PORTION OF THE PRIZE, AND THE VETERAN WILL BE SOLELY RESPONSIBLE FOR THE TAXES ASSOCIATED WITH THIS PORTION OF THE PRIZE. If Grand Prize winner does not designate a US armed service veteran to receive the dental service/cash option, this portion of the prize will not be awarded.
