One (1) Grand Prize: A MOTOROLA XOOM™ with Wi-Fi, a $1000 general use gift card, and a voucher good for one pair of Essilor brand lenses with Crizal Avancé™ with Scotchgard™ Protector, up to a maximum retail value of $500.00. Total approximate retail value (“ARV”) for the Grand Prize: $2,040.00 (actual value may vary based on winner’s prescription and lens; any difference between actual lens value maximum value will not be awarded).

 One (1) Second Prize: A $1000 general use gift card and a voucher good for one pair of Essilor brand lenses with Crizal Avancé™ with Scotchgard™ Protector, up to a maximum retail value of $500.00. Total ARV for the Second Prize: $1,500.00 (actual value may vary based on winner’s prescription and lens; any difference between actual lens value and maximum value will not be awarded).

 One (1) Third Prize: A $500 general use gift card and a voucher good for one pair of Essilor brand lenses with Crizal Avancé™ with Scotchgard™ Protector, up to a maximum retail value of $500.00. Total ARV for the Third Prize: $1,000.00 (actual value may vary based on winner’s prescription and lens; any difference between actual lens value and maximum value will not be awarded).

 If winner is unable to use the lens voucher for him/herself, he/she may transfer the voucher to a friend or family member (but may not sell it). If winner does not wish to use or transfer the lens voucher, no prize will be substituted in lieu thereof. In such case, the winner will still be responsible for taxes on the total ARV for the prize awarded.

 Winner must redeem lens voucher through Sponsor and must provide the following: (i) an empty (no lenses) pair of frames, and (ii) current prescription with eye measurements (written by eye care professional within time period imposed by state in which winner resides). Sponsor will select an Essilor lens based on prescription provided by winner. Winner is responsible for any costs or expenses associated with use of lens voucher, including eye exam fees, practitioner fees, or enhancements to lenses not covered by the voucher. Voucher is subject to all terms and conditions as set forth on the voucher. Voucher must be redeemed within six (6) months of date issued.

 Maximum total ARV for all prizes awarded in the Sweepstakes: $4,540.00.
