1.) Locate one of four gift card images on the www.Forever21.com website; 2.) Click on the gift card image; 3.) Enter the requested information on the entry form. The entry must fulfill all sweepstakes requirements, as specified, to be eligible to win a corresponding prize. Each of the gift card images corresponds to an entry for a chance to win a prize. There are 4 different prizes. Depending on the gift card image, Forever 21 will randomly select a winner or winners from the entries for each of the corresponding gift card images.
i.) Prize 1: $100 Forever 21 gift card = 100 winners;
ii.) Prize 2: $200 Forever 21 gift card = 30 winners;
iii.) Prize 3: $500 Forever 21 gift card = 15 winners;
iv.) Prize 4: $2,100 Forever 21 gift card = 5 winners.
