Grand Prize: (1 winner) Two roundtrip airline tickets valid on flights from city designated by winner to destination designated by winner. Travel must be completed by June 30, 2012 and winner must complete travel arrangements with Sponsor’s agency no later than June 1, 2012. Once travel is ticketed, no itinerary changes will be permitted. Sponsor or its agency will determine prize details, including airline utilized for travel. Winner is responsible for cost of travel insurance (for coverage including, but not limited to delays, health and natural disaster evacuation) and for any required travel documents (i.e., passport). Travel will not be permitted to U.S.-sanctioned countries or destinations where the U.S. government has issued travel warnings for its citizens. Restrictions and blackout dates may apply and travel may be subject to availability. Sponsor shall not be responsible for any flight cancellations, delays, diversions, variations, substitutions, work stoppages, schedule or route changes, or for lost, stolen or misplaced tickets, or any act or omissions whatsoever by the air carriers or any other persons providing any prize-related services. Retail value of both flights not to exceed $4,000. Approximate retail value (ARV) of Grand Prize: $4,000. Actual value dependent upon point of departure and destination of travel. Any difference between ARV and actual value will not be awarded. Sponsor will attempt to notify potential grand prize winner by email or phone at the email address or phone number provided at time of entry, and the grand prize winner will be required to complete, sign and return an Affidavit of Eligibility and Liability and (where legal) Publicity Release which must be received by Sponsor’s agency within five (5) days of the date on the notification or prize will be forfeited and an alternate potential winner may be selected. If potential winner cannot be contacted, fails to complete the required documents, is disqualified, does not meet eligibility requirements or if the prize notification is returned as undeliverable, an alternate potential winner will be selected by random drawing from among all remaining eligible entries. Guest of Grand Prize Winner must sign and return a Liability and (where permitted) Publicity Release within the same time period. If guest is a minor, such documents must be completed, signed and notarized by his/her parent or legal guardian.
