PRIZE AND APPROXIMATE RETAIL VALUES (“ARV”): One (1) grand prize (the “Prize”) will be awarded. The winner will receive Invisalign treatment for his/her named child, who must be named on the official entry form, with an Invisalign provider chosen at Promotion Partner’s sole discretion, with an ARV of up to $5,000. In the event the winner’s named child is deemed ineligible for Invisalign treatment, is not medically suitable, or treatment is not geographically available to winner in Promotion Partner’s sole discretion, he/she will instead receive $1,000, awarded in the form of a check made payable to winner. Invisalign treatment must be initiated within twelve (12) months of winner receipt of Invisalign Voucher, or winner forfeits the prize. Treatment is considered initiated when the selected Invisalign provider ClinCheck® accepts the case.
