Boxes), daytime telephone number (including area code), and date of birth.

Limit one (1) Entry per person per e-mail address per day throughout the Promotion Period.

PRIZES: The approximate retail value (the “ARV”) of the prize is $500.00 and consists of one (1) $500.00 bank card. There is a total of two prize(s) to be awarded to two winners and the total ARV of all prizes awarded is $1,000.00.
