Submit a Story
You must complete and submit the entry form as instructed by sharing What you would do for yourself or someone else if you won $1,000 (the Story). When you provide your Story you will have the option to upload a photo. Stories must be a maximum of 80 characters.

Submit a Video
You must complete and submit the entry form by proving a link to a video that shows What you would do for yourself or someone else if you won $1,000 and is uploaded to the user’s YouTube account (the Video).
