Grand Prize: (1 winner) Cruise trip for four to the Galapagos Islands consisting of roundtrip coach airfare for four persons from major international airport closest to Grand Prize Winner’s residence to major airport closest to port of cruise ship’s embarkation; roundtrip ground transportation between airport and port of embarkation; a 10-day/9-night Celebrity Cruises cruise for four persons to the Galapagos Islands in an ocean view stateroom (two cabins/double occupancy); and $2,000 spending money. Total Approximate Value of Trip: Not to exceed $26,551. If actual retail value is less than stated Total Approximate Retail Value, Grand Prize Winner will not receive difference between actual retail value and Total Approximate Retail Value. All expenses not specified above, such as shore excursions and tours; ship’s physicians and nurses; personal purchases or services, including but not limited to the ship’s gift shops, spa, beauty salon, concessions, non-included food and beverages; mini-bar, entertainment, telephones, souvenirs, gratuities, ground transportation, insurance, personal or spa services, etc., are solely the Grand Prize Winner’s responsibility. All accommodations and reservations will be as arranged by Sponsor and are subject to advance notice, holiday black-out dates and availability. Trip must be completed prior to 12/31/10. Grand Prize Winner and travel companions must possess and present required travel documents including photo identification prior to embarkation and throughout the Trip and travel on same itinerary. Sponsor has no responsibility for lost, stolen, misplaced or mutilated tickets and/or travel documents, baggage, travel delays, or any other occurrences beyond its control. Sponsor has no responsibility for Grand Prize Winner’s inability or failure to accept or utilize the Grand Prize as described herein or for any travel companion’s inability to utilize the Grand Prize. Each Grand Prize Winner’s travel companion must be 21 years of age or older unless (a) Grand Prize Winner is respective travel companion’s parent, legal guardian or spouse or (b) travel companion who is a minor in his/her state of residence is accompanied by his/her parent or legal guardian. The potential Grand Prize Winner will be required to sign and return a notarized Affidavit of Eligibility/Compliance and Publicity/Liability Release, where allowed by law, and such other documents as may be reasonably requested by Sponsor. Failure to sign and return these documents by the return date specified in the prize notification will result in forfeiture of the entire Grand Prize, and, time allowing, the selection of an alternate potential Grand Prize Winner. The Grand Prize Winner’s travel companions must also sign and return a Publicity/Liability Release, as allowed by law, by the stated time period. The Grand Prize Winner will be issued a Form 1099 for tax purposes in the amount of the actual retail value of the Trip and must submit his/her Social Security Number, as required by law. All federal, state and local taxes are the sole responsibility of the Grand Prize Winner. If the Grand Prize is won by a minor in his/her state of residence, his/her parent or legal guardian must sign and return all required documents. Sponsor is not responsible for Winner’s inability to utilize Prize as stated herein. Sponsor may, at its sole discretion, disqualify and select an alternate potential Grand Prize Winner, time allowing, if: (a) the potential Grand Prize Winner fails to respond to notification within the time specified by Sponsor; (b) the return of notification as undeliverable; (c) the potential Grand Prize Winner’s failure to provide Sponsor with satisfactory proof of age, identity and residency; (d) the potential Grand Prize Winner’s failure to execute and return an Affidavit of Eligibility/Compliance (including, without limitation, providing his/her respective Social Security Number for tax purposes) and a Liability/Publicity Release, as allowed by law, by the return date specified by Sponsor, regardless of the cause of such failure; (e) the potential Grand Prize Winner’s failure to be available to complete the trip within the time period set forth herein; (f) the potential Grand Prize Winner’s companions’ failure to execute and return a Liability/Publicity Release, as allowed by law, by the return date specified by Sponsor, regardless of the cause of such failure; or (g) any other non-compliance with the Official Rules. All federal, state and local taxes are solely the responsibility of the Grand Prize Winner. Prior to the award of the Grand Prize, Sponsor reserves the right to require the potential Grand Prize Winner to submit to a confidential background check to confirm eligibility as a condition of awarding the Grand Prize to help ensure that the use of any such person in advertising or publicity for the Promotion will not bring Sponsor & Others into public disrepute, contempt, scandal or ridicule or reflect unfavorably on the Promotion as determined by Sponsor in its sole discretion
