Prize: (1 winner) Round-trip coach class air transportation for two from the major U.S. gateway airport nearest the Grand Prize Winner’s residence to a Los Angeles-area airport; one room, double-occupancy standard hotel accommodations for two at a hotel in Los Angeles, CA, for two nights; round-trip ground transportation between the hotel and the airport; $350 credit redeemable for spa services, at a Los Angeles-area spa to be designated by Sponsor; $300 spending money for the Grand Prize Winner and the opportunity to visit the set of “HawthoRNe”. Should circumstances prevent or limit the opportunity to visit the Show as a part of the Grand Prize, or if filming does not take place after the scheduling of the Grand Prize trip, for any reason, the Grand Prize Winner agrees that the Sweepstakes Entities will have no obligations other than to deliver the remainder of the Grand Prize. The Grand Prize Winner and his/her guest in Sponsor’s sole discretion may also be asked to provide the following information to Sponsor for a criminal background check: name, address, driver’s license number and state, telephone number(s), e-mail address and social security number. The Grand Prize Winner must provide Sponsor with a certified copy of his/her nursing or health care professional credentials at least two (2) weeks prior to Sponsor booking travel. Actual value of Grand Prize may vary based on point of departure and airfare fluctuations. The Grand Prize Winner is responsible for all costs and expenses not expressly provided for above, including, without limitation, additional ground transportation, meals, incidentals, tips, telephone calls, souvenirs, other personal expenses and hotel charges incurred during the trip. In the event the Grand Prize Winner is a minor under the law of his/her state of residence, his/her guest must be his/her parent or legal guardian. All travel arrangements must be made through Sponsor’s agent on a carrier of Sponsor’s choice and trip must be booked a minimum of thirty (30) days in advance. Dates are subject to availability and certain black-out dates and other travel restrictions may apply. Sponsor reserves the right to approve any travel dates and trip must be taken by July 10, 2009.
