follow the instructions to complete and submit the official contest entry form. This form will require the entry of the Entrant’s name, age and contact information, along with the three “players” (Terminix Service Professionals) that the Entrant selects for his or her team. Entrants may not change their team at any point throughout the Contest Period; the three “players” selected in the entry form will not and cannot change for Entrants for the entirety of the Contest. The completed and submitted form will operate as an entry for a chance to win a Big Game Giveaway.
Limit (1) entry per person during the Entry Period.
Contest Winner Determination. Administrator will keep a grand total of points for all Entrants. The ten (10) “players” will be assigned points based on total customer calls requesting Terminix Rodent and/or Wildlife Exclusion services received by the state that they represent in ten (10) point increments (one hundred (100) points will be assigned to the “player” from the state that receives the most calls; whereas ten (10) points will be assigned to the “player” from the state with the fewest calls). The Entrant whose team gained the most points during the Contest Period will be deemed the potential winner, subject to verification by Sponsor that such Entrant also meets all of the eligibility, proof of identity and other requirements as may be outlined by Sponsor. In the event of a tie, a random drawing will be conducted from all eligible entries received to determine one (1) winner. Potential Winner will be determined on January 7, 2015.
Prize. One (1) prize winner will receive a Big Game Giveaway package which includes the following items to host the ultimate viewing party for “The Big Game in Arizona in February 2015,” including: one (1) 70″ SHARP AQUOS Q+ Series HDTV (Model: LC-70TQ15U), one (1) Bose® CineMate® 130 Home Theater System (Model: CineMate 130), two (2) La-Z Boy James Collection Rocker Recliners (SKU 5213440) and an American Express gift card for catering services at the party (maximum total value of $500). Total Prize Approximate Retail Value (“ARV”): Five Thousand and 00/100 Dollars ($5,000.00) or less.
