An individual may enter the Sweepstakes by submitting a photo of the place where he/she participated in their activity, along with a short description. Photo must be a jpg or gif format, under 2 MB file size and written descriptions may not exceed 30 words.

For entry into the Sweepstakes without completing either of the actions above, mail a 3″ x 5″ card containing your complete name, address, e-mail, phone number and date of birth in an envelope to: UnitedHealthcare Promotion Department, Summer 2013 Source4Women‘We Dare You to Get 30 Minutes of Activity Today Sweepstakes’, 9700 Health Care Lane, MN017-E500, Minnetonka, MN 55343-9026. Mail-in entries must be postmarked by September 30, 2013 and received by October 4, 2013.

Entry is limited to once a day.

One (1) winner will be drawn at 12:00:00, noon (EST) on Friday each week by the Administrator, at random, from among all eligible entries received up until the drawing time. Drawing dates are as follows: September 6, 13, 20, and 27. Odds of winning depend on the number of eligible entries received throughout the Entry Period, up until the drawing time. Non-winning entries will be carried forward for subsequent weekly drawings. Weekly winners remain eligible to win the Grand Prize.

On or about October 4, 2013, the Sponsor will conduct a random drawing from among all eligible entries received during the Entry Period to select one (1) entry as the Grand Prize winner.

WEEKLY PRIZES (4): Each weekly prize winner will receive a $25.00 Visa® Gift Card. 4 (four) total weekly prizes will be awarded in the Sweepstakes and the approximate retail value (ARV) of all weekly prizes awarded in the Sweepstakes is: $100.00 USD. An entrant may only win one (1) weekly prize throughout the entire Sweepstakes.

ONE (1) GRAND PRIZE WINNER: On or about October 4, 2013, one (1) Grand Prize winner will be drawn, at random, from among all eligible entries received throughout the Entry Period. The Grand Prize winner will receive a $400 Visa® Gift Card. The approximate retail value (ARV) of the Grand Prize is $400.00 USD.
