PRIZE: One (1) Grand Prize: A trip for the Winner and one (1) verified travel guest (“Guest”) to New York City for a makeover and shopping spree for the Winner.
Grand Prize includes: round-trip coach transportation from a major commercial gateway airport near Winner’s residence within the United States to New York, New York, transportation to and/or from the airport and the hotel, two (2) days/two (2) nights hotel accommodations (double occupancy accommodations), hair appointment for the Winner, spa appointment for the Winner, a shopping spree awarded as a $3,000 American Express gift card, two (2) dinners for two (2), and a $1,000 American Express gift card to be used for travel incidentals. Approximate Retail Value (“ARV”) of Grand Prize is $9,700.
In addition, if the Winner meets certain criteria, a CoolSculpting® procedure will also be offered to the Winner. In order for the Winner to receive the CoolSculpting® procedure, the Winner will be required to have a free consultation with a CoolSculpting® Center (“Provider”) to ensure the Winner is a qualified candidate for the procedure. Provider for consultation and free procedure will be chosen at the sole discretion of Sponsor. Winner may have to travel depending on the location of the nearest CoolSculpting® Provider. Grand Prize Winner is responsible for all costs and expenses associated with the travel to the CoolSculpting® Provider. Winner will have ninety (90) days to receive a consultation and six (6) months to redeem CoolSculpting® procedure offer once a CoolSculpting® Provider determines he/she is qualified candidate for the procedure. Only a licensed physician with the network of Providers has the ability to determine and prescribe the procedure, and all decisions of that physician are final and binding. The maximum retail value of the CoolSculpting® procedure appointment is approximately $1,200, but the actual value will vary depending on the region in which the winner lives.
