During the Submission Phase, visit http://www.gnc.com/MeOnGNC and
 follow the links and instructions to complete and submit the registration form, including
 a valid home address. P.O. Boxes are not permitted. Then, follow the directions to
 submit your submission(s) into one (1) or both of the two (2) prize pools.
 1. Photo Submission: Follow the links and instructions to submit one (1) photo
 portraying your own story about health goals, a description of the photo (100 words
 or fewer) and a title of the photo (not to exceed 10 words), including the GNC
 graphic (asset) (provided for download on the site), collectively referred to as your
 photo submission (the “Photo Submission”) in order to receive one (1) Contest
 entry into the Photo Submission prize pool.
 2. Video Submission: Follow the directions to upload one (1) video portraying your
 own story about health goals, a description of the video (100 words or fewer) and a
 title of the video (not to exceed 10 words), including the GNC video clip

 Limit: Each entrant may enter each prize pool one (1) time during the Submission
 Phase.

 The Sweepstakes: During the Voting Phase, follow the links and instructions as set
 forth in Section 5(a) above to register for the Promotion. You will be prompted to vote
 for your favorite finalist in one (1) or both categories. You will earn one (1) entry into
 the Sweepstakes for each category voted for. Limit: Each entrant may receive up to
 two (2) entries into the Sweepstakes per day during the Promotion Period

 Judging Phase: During the Judging Phase, qualified judges determined by
 Sponsor in its sole discretion will select the top fifteen (15) Video Submissions and
 the top fifteen (15) Photo Submissions (each a “Finalist”) based on the following
 criteria (“Judging Criteria”):
  Creativity/Originality (25%)
  Quality of Submission (25%);
  Use of GNC Respect Graphics (Assets) (25%); and
  Fit with Contest Theme (25%).

 . Limit: Each entrant may vote one (1) time per day during the Voting Phase.

 Prizes:
 a. Contest: TWO (2) FIRST PRIZES (ONE (1) EACH FOR PHOTO/VIDEO): $25,000.
 TWO (2) SECOND PRIZES (ONE (1) EACH FOR PHOTO/VIDEO): $15,000. TWO (2) THIRD PRIZES (ONE (1) EACH FOR PHOTO/VIDEO): $5,000.
 b.
 Sweepstakes: ONE HUNDRED (100) SWEEPSTAKES PRIZES: $100. Odds of winning a
 Sweepstakes Prize depend on the number of eligible entries received during the Voting
 Phase.

 Limit: One (1) Sweepstakes Prize and one (1) Contest Prize per person. Total approximate retail value of Sweepstakes Prizes: $10,000.
