This is nothing but a fraying scrap of fabric abou 6″ x 2.5″. Not worth the time to fill it out. Also, the same day I got this, I got another envelope from the same ministry telling me he had a prophecy about me and seeking donations to find out what it is…CREEPY mailing list I got myself onto just for something free…LOL
