PRIZES: Five (5) prizes will be awarded, each consisting of a four (4)-day/three (3)-night trip
to a top health and wellness retreat located in the continental U.S. to be chosen from several
sponsor-selected destinations. Trip package includes: (i) Roundtrip coach class air
transportation for winner and one (1) guest from the major airport nearest the winner’s
residence to the major airport nearest spa; (ii) ground transportation to and from airport and
spa; (iii) four (4) days/three (3) nights standard double-occupancy accommodations; (iv) a
selection of spa services; (v) meals; (vi) $2,000 spending money awarded in the form of a
check made payable to the winner. ARV: up to $15,100. ARV of prize may vary depending on
spa selection, point of departure, travel dates and fare/rate fluctuations; any difference between
the stated ARV and final value of the prize will not be awarded. Total ARV of all prizes: up to
