To enter the Sweepstakes, go to the UnitedHealthcare ‘We Dare You’ website at http://www.wedareyoutoshare.com/puzzle. An individual may enter the Sweepstakes by completing a picture using puzzle pieces of UnitedHealthcare Tools. Upon completion of the game regardless of the score the participant will earn 1 entry. Entry is limited to once a day. Weekly winners remain eligible to win the Grand Prize.

WEEKLY PRIZES (4): Each weekly prize winner will receive a $25.00 Visa® Gift Card. 4 (four) total weekly prizes will be awarded in the Sweepstakes and the approximate retail value (ARV) of all weekly prizes awarded in the Sweepstakes is: $100.00 USD. An entrant may only win one (1) weekly prize throughout the entire Sweepstakes.

ONE (1) HIGH SCORE WINNER: On or about June 6, 2016, one (1) High Score Prize winner will be drawn, based on the highest cumulative score, from among all eligible entries received with the highest score throughout the Entry Period. The High Score Prize winner will receive a $100 Pre-Paid Gift Card. The approximate retail value (ARV) of the High Score Prize is $100.00 USD.

ONE (1) GRAND PRIZE WINNER: On or about June 6, 2016, one (1) Grand Prize winner will be drawn, at random, from among all eligible entries received throughout the Entry Period. The Grand Prize winner will receive a $400.00 pre-paid gift card. The approximate retail value (ARV) of the Grand Prize is $400.00 USD.

To enter the Sweepstakes, go to the UnitedHealthcare ‘We Dare You’ website at http://www.wedareyoutoshare.com/see. An individual may enter the Sweepstakes by answering the poll question about using your health care plan benefits on the See & Sound Off page of the site. Each day they may answer the poll question. Each time the poll question is answered, it is considered to be one (1) entry. Entry is limited to once a day. Weekly winners remain eligible to win the Grand Prize.

A person may gain an additional entry once they have answered the poll question, per the instructions above, and then on the Thank You page click on a Resource article or video that is presented along the right hand side of the Thank You page.

An individual may enter the Sweepstakes by answering a random question presented on the site about using the prescription benefits of your health care plan. Answering the question incorrectly earns the participant 1 entry. Answering the question correctly earns the participant 5 entries. Entry is limited to once a day. Weekly winners remain eligible to win the Grand Prize.
