PRIZE AND APPROXIMATE RETAIL VALUES (“ARV”): One (1) grand prize (the
“Prize”) will be awarded. The winner will receive Invisalign treatment for his/her named
child, who must be named on the official entry form, with an Invisalign provider chosen
at Promotion Partner’s sole discretion, with an ARV of up to $5,000. In the event the
winner’s named child is deemed ineligible for Invisalign treatment, is not medically
suitable, or treatment is not geographically available to winner in Promotion Partner’s
sole discretion, he/she will instead receive $1,000, awarded in the form of a check made
payable to winner. Invisalign treatment must be initiated within twelve (12) months of
winner receipt of Invisalign Voucher, or winner forfeits the prize.
