the form is in flash, it takes a second or two to load. Make sure you have flash installed as well

A few people have gotten this party pack but the CD says [has to be returned to record company upon demand]. I have not gotten mine here in AZ, but that will be a major bummer if we have to pay postage and take to post office to send back. Wierd.
